This application is for vehicles owned and / or managed by the Washington State Department of Enterprise Services (DES) Fleet Operations.
Any vehicles that are permanently assigned to a person or program AND need to track utilization for agency internal billing purposes should use this form. It allows multiple billing codes per trip / usage ticket.
Please enter all requested information and double check your entry prior to submitting.
A detailed report of all vehicle usage for your agency will be provided with your agency's monthly Fleet Operations invoice.
