Accident Info |
The Accident Info tab enables you to record and modify information about accident (or accidents) associated with an incident. The data collected includes the primary and contributing causes, agencies involved, location, and injuries and fatalities incurred.
Note: An incident must be of type ACCIDENT in order for you to use this screen.
Tip: If an incident is closed, it cannot be modified. You must change the status to something other than CLOSED before you can modify the data.
Tip: To limit the display of information in the grid and maximize performance, use the filter. You can sort on incident ID, type, status, equipment ID, symptom, responsibility, a range of occurred dates and reported dates, and trip ID, among other values.
The fields in the top section of the Accident Info tab display the incident ID, the name of the user who recorded the incident, and the incident type. These fields are display only.
Incident ID
System-assigned identifier for the incident.
Entered by
Identification code of the user who initially entered the incident. The user's name displays in the unlabeled field to the right.
View details
This incident management screen displays all types of incidents. To view details specific to an incident that has a type other than INCIDENT, click the View details button. The appropriate incident screen will appear. For example, if the incident type is UNUSUAL OCC, you can click the View details button to access the Data -> Incident Management -> Unusual Occurrences screen for the incident.
Incident type
Overall category for the incident. The field has a list box:
INCIDENT -- default
DELAY
UNUSUAL OC
ACCIDENT
INJURY
ALARM
The fields in the bottom section of the tab enable you to provide additional information about the accident. All fields are optional.
Primary cause
Primary cause of the accident. This field has a list box. The cause's text description displays beneath the Primary cause field. (Accident causes are defined on the Data -> Incident Management -> Setup -> Causes screen.)
Contributing cause
Contributing cause of the accident. This field has a list box. The cause's text description displays beneath the Contributing cause field.
Reporting agency
Free-form text field in which you can type the name of the agency to which the accident was reported. The maximum field length is 30 characters.
Responsible agency
Free-form text field in which you can type the name of the responsible agency to which the accident was reported. The maximum field length is 30 characters.
Other agency
Free-form text field in which you can type the name of another agency to which the accident was reported. The maximum field length is 30 characters.
Location ID
identifier of the location where the accident occurred. This field has a list box. The location's name displays in the unlabeled field to the right.
Tip: If you select a location ID, the Address and Line 2 fields auto-populate with the location's physical address.
Address ID
Identifier of the address used for the accident. If an address ID is entered, the address ID information will be used instead of address lines 1-4. Detailed address information for address IDs is set up on the Addresses screen.
Address
Physical address where the accident occurred. If the location is not on file, type the address. Maximum field length is 30 characters.
Line 2
Additional line for location's physical address, if needed. Maximum field length is 30 characters.
Site
Free-form text field in which you can type information about the accident site. Maximum field length is 50 characters.
The fields in the Injuries and fatalities screen group in the bottom section of this tab specify the individuals involved in the accident and the severity of their injuries.
Delete
If this field has a check, the entire row is deleted when you click Save.
Severity
The severity of the injury. This field has a list box with two choices: INJURED and DECEASED.
Person type
The injured person's role in the accident. This field has a list box with the following choices:
CREW
CIVILIAN
EMPLOYEE
FIRE
PASSENGER
POLICE
Name
Name of the injured person. Maximum field length is 30 characters.
Comment
Free-form text field in which you can type comments about the injury.
