Injury Info |
The Injury Info tab enables you to record and modify information about the individual (or individuals) injured in an incident. The data collected includes the person's contact information, extent and status of the injury, the person's role (i.e., employee, passenger, police), and the number of work days lost.
Note: If an incident is closed, it cannot be modified. You must change the status to something other than CLOSED before you can modify the data.
Tip: To limit the display of information in the grid and maximize performance, use the filter. You can sort on incident ID, type, status, equipment ID, symptom, responsibility, a range of occurred dates and reported dates, and trip ID, among other values.
The fields in the top section of the Injury Info tab display the incident ID, the name of the user who recorded the incident, and the incident type. These fields are display only.
Incident ID
System-assigned identifier for the incident.
Entered by
Identifier of the user who initially entered the incident. The user's name displays in the unlabeled field to the right.
View details
This incident management screen displays all types of incidents. To view details specific to an incident that has a type other than INCIDENT, click the View details button. The appropriate incident screen will appear. For example, if the incident type is UNUSUAL OCC, you can click the View details button to access the Data -> Incident Management -> Unusual Occurrences screen for the incident.
Incident type
Overall category for the incident. This field has a list box:
INCIDENT -- default
DELAY
UNUSUAL OC
ACCIDENT
INJURY
ALARM
The fields in the bottom section of the tab enable you to provide additional information about the injured individual (or individuals). All fields are optional.
Employee ID
Identification number of the injured employee. This field has a list box. If the injured person is not an employee, leave this field blank. (Employee IDs are assigned on the Personal Info tab of the Data -> Shop Activity -> Setup -> Employees -> Primary Information screen.)
Tip: If you specify an employee ID, the Name, Address, Line 2-3-4, and Phone fields populate automatically with the employee's contact information.
Name
Name of the injured person. If the injured person is not an employee, type his or her name. Maximum field length is 30 characters.
Address
Mailing address of the injured person. If the injured person is not an employee, type his or her address. Maximum field length is 30 characters.
Line 2
Additional line for mailing address, if needed. Maximum field length is 30 characters.
Line 3
Additional line for mailing address, if needed. Maximum field length is 30 characters.
Line 4
Additional line for mailing address, if needed. Maximum field length is 30 characters.
Phone
Area code and telephone number. Maximum field length is 30 characters.
Cause
identifier of the incident's cause. This field has a list box. The cause's description displays in the unlabeled field below the Cause field. (Causes of incidents are defined on the Data -> Incident Management -> Setup -> Causes screen.)
Description
Free-form text field in which you can describe the injury. Maximum field length is 50 characters.
Status
The status (whereabouts) of the injured person. This field has a list box.
Injury type
The type of injury sustained by the individual. This field has a list box with the following choices:
BROKEN BONE
CUT/ABRASION
BURN
PUNCTURE WOUND
Type of person
The role of the injured individual in the incident. This field has a list box with the following choices:
EMPLOYEE
PASSENGER
POLICE
ATTENDANT
CREW
Case number
The case number assigned to the injury. Maximum field length is 30 characters.
Number of days
Lost
Number of days that the individual was unable to work because of the injury. Maximum field length is five characters.
Restricted duty
Number of days that the individual was on limited work duty because of the injury. Maximum field length is five characters.
First aid only
If this field has a check, the injured person only required first aid.
Fatal
If this field has a check, the injury resulted in death.
