Change request ID
Identifier of the change request that you are creating, or that is currently selected in the grid.
Status:
Change Request Statuses are user-defined statuses that indicate that this change request is either PENDING or COMPLETED.
View changes button
This button opens the asset source screen for the change request. You must have rights to the target screen for the source screen to open. The screen displays changed fields as highlighted fields.
If you only have view rights to the target screen, this screen launches in View mode.
If you do not have view rights to the target screen of the Change Request record, this button will not open the source screen.
Apply changes button
This button allows you to apply the changes on the source screen. If you have edit rights to the target screen, this screen launches in Edit mode to the correct data record. The requested changes are applied to the screen, and fields are highlighted where changes have been applied. This allows you to quickly review and save the requested changes on the source screen.
Description
This field is for a description of what this change request is for.
Screen ID
This is the identifier of the screen that initially created the change request. This field is display only. If the screen ID is entered or changed, and the Notification Template drop-down below is empty, the system loads the template ID from the External Data Mapping page if there is an entry for that ID.

Action type
The type of action you are taking on this change request.
INSERT: Insert a new data record
UPDATE: Update an existing data record
Type
The type of change request.
Key values
Identifying information about the row that was inserted or updated by the change request selected in the grid. This field is display only.
Change requested by
The person that requested the change.
at
The date the time the change was requested. The date and time is auto-populated when the change request is made.
Requested from screen ID
The screen ID from where the change request was requested.
Requested from key values
The key values for the change request.
Approved
If this field has a check, the change request is approved. You must specify a valid user ID in the Change approved by field.
Change approved by
Identifier of the user who approved the project by placing a check in the Approved field. This field has a list box.

Notification template

Template screen. This field is optional.
Note: If a template ID is provided, a notification instance ID is automatically generated based on the template data and recorded on the change request. An instance ID is generated only if the change request does not have one already.
Notification templates are defined on the Data -> Shop Activity -> Setup -> Notification Template screen.
Notification status
Status of the notification. This field has a zoom
button to the Notification
Instance screen. This field is display only.
Date and time completed
The date and time the change request was completed.
